Eligibility

  • All full-time employees who work 30 regularly scheduled hours a week are eligible for benefits
  • 30-days waiting period for new hires
  • Benefits go into effect the day after your waiting period.
  • If you do not enroll within the 30 day waiting period, you will not be able to enroll for benefits until the next open enrollment period or until you have a life event that would support an enrollment.

Spouses

  • Spouses who are offered medical coverage from their own employer will not be eligible to participation the medical plan
  • Must be legally married

Eligible Dependents

  • Birth child, stepchild, adopted child or foster child of the insured or covered employee. Please note, you or your covered spouse must be under at 26. You or your covered spouse must be the legal guardian and the dependent must be under the age of 26, unless they are an adult child who is disabled, and totally dependent on the employee for support. 
  • If adding a dependent child, employees will be required to submit a birth certificate, copy of most recently filed federal income tax return, or qualified medical child support/court paperwork.
  • Wellness Program Requirements: Employees hired between November 1, 2024, and August 31, 2025, and eligible spouses (if applicable), will need to complete the wellness program activities in order to receive their medical premium credit for 2026. If you have already completed any of these activities with a previous employer, please provide proof of completion (such as an EOB) for each activity. Please note employees and spouses earn medical credits separately.

    Employees hired on September 1, 2025, or later are not required to complete the activities by October 31, 2025 and will automatically receive their credit.
