NEW HIRE
Eligibility
- All full-time employees who work 30 regularly scheduled hours a week are eligible for benefits
- 30-days waiting period for new hires
- Benefits go into effect the day after your waiting period.
- If you do not enroll within the 30 day waiting period, you will not be able to enroll for benefits until the next open enrollment period or until you have a life event that would support an enrollment.
Spouses
- Spouses who are offered medical coverage from their own employer will not be eligible to participation the medical plan
- Must be legally married
Eligible Dependents
- Birth child, stepchild, adopted child or foster child of the insured or covered employee. Please note, you or your covered spouse must be under at 26. You or your covered spouse must be the legal guardian and the dependent must be under the age of 26, unless they are an adult child who is disabled, and totally dependent on the employee for support.
- If adding a dependent child, employees will be required to submit a birth certificate, copy of most recently filed federal income tax return, or qualified medical child support/court paperwork.
